SECTION III - DISCLOSURE AND CONSENT

PARENT / GUARDIAN CONSENT

I / We the Parents / Guardians of the above-named athlete hereby give my / our approval to the athlete’s participation in any and all KO Bros Sports Academy (KOA) activities during the duration of Summer. I / We assume all risks and hazards incidental to such participation, including transportation to and from such activities. I / We do hereby waive, release, absolve, indemnify, and agree to hold harmless the KO Bros Sports Academy (KOA), including its sponsors, partners, the city and county in which KOA resides and other related participants, for any injury to my / our child.

RULES AND REGULATIONS

I / We as Parents / Guardians of said athlete understand it is the responsibility of the Parent / Guardian and athlete to comply with any and all rules and regulations of KO Bros Sports Academy (KOA). Any noncompliance with rules and regulations shall be cause for dismissal or suspension from all future KO Bros Sports Academy (KOA) sanctioned events.

SECTION IV - SOCIAL MEDIA RELEASE

PURPOSE

Social media offers the opportunity for people to gather in online communities of shared interest and create, share or consume content. KO Bros Sports Academy (KOA) recognizes the benefits of social media as an important tool of engagement and enrichment for our customers. It is important that the reputation of the KOA and its affiliated associations are not tarnished by anyone using social media tools inappropriately, particularly in relation to any content that might reference KOA. When someone clearly identifies their association with KOA, and/or discusses their involvement in the organization in this type of forum, they are expected to behave and express themselves appropriately and in the ways that are consistent with the KOA values and policies. This policy aims to provide some guiding principles to follow when using social media. This policy does not apply to the personal use of social media platforms by customers or staff that makes no reference to the KO Bros Sports Academy or related issues.

SCOPE

This policy covers all forms of social media.

Social media includes, but is not limited to, activities such as: Maintaining a profile page on social or business networking sites (such as LinkedIn, Facebook, Instagram, Snapchat, or Twitter); Content sharing including Flicker (photo sharing), DropBox, and YouTube (Video Sharing); Commenting on blogs for personal or business reasons; Taking part in conversations on public and private web forums (message boards); or Editing a Wikipedia page. The intent of this policy is to include anything posted online where information is shared that might affect customers, employees, sponsors or KO Bros Sports Academy (KOA) as an organization and the reputation of the company. In consideration of my child / athlete being allowed to participate, in any way, at the KO Bros Sports Academy (KOA) and any other official events and activities, the undersigned agrees that KOA is hereby granted the unrestricted right and permission, free from approval or review, to copyright and/or use my child's/ward's likeness in all media now or hereafter known, including but not limited to, pictures and videos of my child which he/she may be included intact or in part for promotion or other commercial use. Failure to comply with this policy may be cause for the removal of my child(ren) from the KO Bros Academy (KOA) program.

SECTION V - FINANCIAL AID

At KO Bros Sports Academy (KOA), we are committed to ensuring access for families of all economic backgrounds through our Financial Aid program. This tuition assistance is available exclusively for our KOA Elite Package, which includes comprehensive services such as Learning Center access, General Training, Position-Specific Training, our KOA Meal Plan and Nutrition Program, and Ministry services. We strive to make high-quality athletic and academic development attainable for all dedicated student-athletes.

Financial Aid Acknowledgement

If approved for enrollment in the 2025 KOA Summer Program, a KOA Team Member will contact me to schedule an in-person or virtual meeting to finalize and confirm my family's Financial Aid accommodation.

$125 NON-REFUNDABLE REGISTRATION FEE ACKNOWLEDGEMENT

By selecting this box, I acknowledge that if my application is approved, I am required to pay a NON-REFUNDABLE KOA Registration Fee of $125 to fully reserve my spot in the KOA Enrollment Class of 2025-2026.

Upon approval, you will receive a payment link to process the $125 NON-REFUNDABLE REGISTRATION FEE, securing your place in the 2025 Summer Program. Once your payment is successfully processed, a KOA team member will reach out to assist you in completing your athlete's KOA Medical Release Form for the 2025 Summer.
